Prehospital lactate clearance is associated with reduced mortality in patients with septic shock

چکیده

Assessment of disease severity in patients with septic shock (SS) is crucial determining optimal level care. In both pre- and in-hospital settings, blood lactate measurement broadly used combination the clinical evaluation as picture alone not sufficient for assessing outcomes. From 15th April 2017 to 2019, SS requiring prehospital mobile Intensive Care Unit intervention (mICU) were prospectively included this observational study. Prehospital clearance was estimated by difference between (time first contact mICU prior any treatment) (at hospital admission) levels divided lactate. Among 185 study, missing six (3%) setting four (2%) at admission, thus 175 (95%) analysed (mean age 70 ± 14 years). Pulmonary, digestive urinary infections probably cause respectively 56%, 22% 10% cases. The 30-day overall mortality 32%. Mean significantly different who died those survived (respectively 0.41 2.50 mmol.l?1 vs 1.65 2.88 mmol.l?1, p = 0.007). Cox regression analysis showed that associated > (HRa [CI95] 0.49 [0.26–0.92], 0.028) < 2.04 [1.08–3.84], 0.028). A increase handled mICU. Further studies will be needed evaluate if or combined scores could affected triage decision-making process patients.

Is lactate clearance impaired in septic shock?

In a recent article in Critical Care, Tapia and colleagues evaluated the relationship between exogenous lactate clearance and liver perfusion in a short-term model of endotoxemic shock [1]. The authors concluded that the very low portal-hepatic vein lactate gradient was indicative of the inability of the liver to metabolize the increased lactate load. Although we believe that the authors addres...

Early lactate clearance is associated with improved outcome in severe sepsis and septic shock.

OBJECTIVE Serial lactate concentrations can be used to examine disease severity in the intensive care unit. This study examines the clinical utility of the lactate clearance before intensive care unit admission (during the most proximal period of disease presentation) as an indicator of outcome in severe sepsis and septic shock. We hypothesize that a high lactate clearance in 6 hrs is associate...

Lactate clearance is associated with mortality in septic patients with acute kidney injury requiring continuous renal replacement therapy

The aim of the study was to assess the clinical utility of lactate measured at different time points to predict mortality at 48 hours and 28 days in septic patients with acute kidney injury (AKI) requiring continuous renal replacement therapy (CRRT).Consecutive critically ill patients with septic AKI requiring CRRT were prospectively studied. Variables were collected at initiation of CRRT and 2...

Low-dose immunoglobulin G is not associated with mortality in patients with sepsis and septic shock

BACKGROUND The administration of low-dose intravenous immunoglobulin G (IVIgG) (5 g/day for 3 days; approximate total 0.3 g/kg) is widely used as an adjunctive treatment for patients with sepsis in Japan, but its efficacy in the reduction of mortality has not been evaluated. We investigated whether the administration of low-dose IVIgG is associated with clinically important outcomes including i...

Underweight is associated with mortality in patients with severe sepsis and septic shock

Methods A retrospective analysis of prospectively collected database of patients with severe sepsis and septic shock was performed. We classified the patients into four groups according to the basic criteria of the World Health Organization (WHO) guidelines as follows, underweight, < 18.5; normal, 18.5-24.99; overweight, 25-29.99; obesity, ≥ 30 kg/m, respectively. The multivariate logistic regr...
